Contact the CHIC office.  If there is a duplicate entry, MDH will merge the records.

MIIC inactivates your account after 60 days of non-use so you will need to contact your site administrator to reset your password or contact the CHIC.

Is there any cost for participating in MIIC?

No, MIIC is provided as a public health service to prevent disease and is funded through federal, state, and local public funds. Your critical contribution is to get your complete and accurate immunization data into MIIC in a timely way.
